How Do We Collect Information And What Information Do We Collect?

When you visit our website, we may collect information from you when you register for a user account on our site, enroll in Income Assist®, submit a claim, review the status of a claim, fill out a form or survey, communicate with us via email, perform maintenance activities on your account such as updating your contact information, close a claim or cancel your enrollment in Income Assist®. The information we collect may include your name, address, phone number, email address, date of birth, Income Assist® policy or certificate of insurance number, social security number, insurance premium billing information, the date of loss or the event date, and any information you provide when registering on our site or submitting a claim.

In addition, depending upon the type of claim you are requesting, we will collect some personal information in order to process your claim.

Website Tracking Technology

We maintain a record of all information collected on this site for the period of time required by applicable law, including a "click history" of the internal pages that you visit while on this site. Your click history is only maintained for the purpose of conducting internal web analytics, audits and diagnostics on the site, and will not be shared for any other purpose.

  • when we have reason to believe that such disclosure is necessary to identify, contact or bring legal action against someone who may be causing injury to or interference with (either intentionally or unintentionally) our rights or property, customers or other users of this website, or anyone else who could be harmed by such activities.

Misrepresenting, obscuring, suppressing, or replacing your identity on this site is forbidden. If information is accessed accidentally or in error on this site which is not intended for your utilization, any dissemination, distribution, or copying of such information is prohibited. You should not share your claim number with anyone. You have the right to submit a claim or review your claim status in non-electronic format.
